Sixteen items, four hidden groups of four, and the whole difficulty is that several items look like they belong somewhere else — only one arrangement of all sixteen works. Pick four and submit; a correct group locks with its name revealed, a wrong one costs a life, and you have four. Three right out of four is called out as one away rather than left ambiguous.
